JURY QUESTIONNAIRE — RESPONSE REQUIRED
If you have been selected as part of the Jury Pool for the Mashpee Wampanoag Tribal Court, your Jury Questionnaire is an important part of the jury selection process and must be completed and returned as required by law.

CHAPTER 3 — SECTION 4(d): Any person who fails to return a completed juror questionnaire may be summoned by the District Court to appear to complete such a form. Any person who fails to appear as directed or willfully misrepresents a material fact on the juror questionnaire to evade jury service may be ordered by the District Court to appear and show cause as to why they should not be sanctioned and may be subject to any of the sanctions listed in Subsection 6 of this Chapter 3.
Please complete, sign, and return your Jury Questionnaire by 4pm on September 28th, 2026.
Failure to return the questionnaire may result in further action as provided in Section 4(d).
Your prompt cooperation is important to the proper administration of the Tribal Court and the jury selection process.
