The Massachusetts Department of Public health (MDPH) has launched a Covid-19 Community Impact Survey. Tribal members ages 14 and over in the state of Massachusetts can take the survey.

By taking the survey you will have a profound impact on how services are deployed to our tribal community. The Community Impact Survey will help MDPH learn about the needs of tribal members across the state of Massachusetts so it can plan resources and support around Covid-19.
The survey takes about 10-15 minutes to finish. They will not ask for your name and your answers will be kept completely private. Please take the survey and let MDPH know how they can support tribal members.
