Massachusetts Disability Community Health Needs Assessment

The questions on this survey were selected by the Health and Disability Partnership at the Department of Public Health (DPH). They were reviewed by self-advocates with disabilities.

This survey will allow the Health and Disability Program at DPH to understand more about what life is like for people with disabilities and what affects people's health.

Community Health Equity Survey

The Community Health Equity Survey (CHES) aims to help communities address barriers to health. The Massachusetts Department of Public Health will use survey results to improve their programs, make decisions about funding and resources, and support policies to improve health inequities. They will also share the results of the survey with partners across Massachusetts to support the important work to improve health in Massachusetts communities.
