Transitional Housing and Shelter Program Application
ALL APPLICATIONS WILL BE SCREENED USING A “POINT SYSTEM” TO DETERMINE HOUSING VULNERABILITY AND PLACEMENT

Transitional Housing and Shelter Program Eligibility Requirements:
The following eligibility requirements are as follows for participation in the Transitional Housing and Shelter Program.
Be an enrolled member of the Mashpee Wampanoag Tribe.
Eighteen years old or (legally) emancipated minor.
Families with children under the age of 18.
*MWT children under the age of two years qualify nontribal head of household pending ICWA involvement.
Veteran
Elder 55+
Currently living on the streets, in a vehicle, in an emergency shelter or couch surfing.
Currently homeless because of domestic violence, sexual assault, dating violence or stalking.
Transitional Housing and Shelter Program Criteria:
The following criteria is mandatory to actively participate in the Transitional Housing and Shelter program:
Have a valid ID or other form of document confirming you are 18 years of age or older.
Provide verification of health insurance or apply within one week of entering the program and agree to remain insured while in the Transitional Housing and Shelter program.
Agree to receive nursing services for medication management (shelter clients only).
Be willing to seek and/or accept assistance involving medical or mental health services.
Be willing to abstain from illegal drugs and unprescribed drugs.
Agree to a drug test on the day of acceptance into the program, including random screens.
Be willing and able to sustain a monthly occupancy fee of up to $743 per person shared (studio), $1,486 for single (studio), $1,041 shared (two bedroom) and $2,082 single (two bedroom)

All fees must be submitted in the form of a money order.
Be willing to participate in a structured savings program within 30 days of the program (shelter clients only).

Be willing to apply for low-income housing and to accept Permanent Supportive Housing as soon as it comes available.
Commit to goals set in conjunction with the Transitional Housing and Shelter Program and the Mashpee Wampanoag Tribe Housing department to achieve financial stability, interpersonal skills, positive social connections, and to obtain Permanent Supportive Housing.
Demonstrate an ability to work cooperatively with others as a participating tribal member by showing that you embrace your culture and respect your people.
